📄 Dear Dream Beauty｜半永久服務同意書

預約與使用 Dear Dream Beauty 任何服務，即代表您已閱讀、理解並同意以下條款。

一、個人資料與狀況告知

預約與接受本店服務，即表示您確認並保證以下事項為真實、完整且未隱瞞：

1. 您已誠實告知自身皮膚狀況、過敏史、既往紋繡紀錄、健康狀態等相關資訊。

2. 如有任何疾病、過敏體質、凝血功能異常、蟹足腫、糖尿病、甲狀腺疾病、重大疾病等可能影響施作之情況，您已主動告知本店。

3. 您確認施作當日並非懷孕、哺乳期或月經期間。

4. 若因隱瞞身體狀況導致之任何反應、感染或不良後果，需自行承擔，本店不負任何法律責任。

二、服務與付款條款

1. 本店所有服務屬美容性質，效果會依個人體質、皮膚狀況、代謝、作息等不同而有所差異。

2. 本店僅於收到全額款項後開始施作。未付款前不得要求進行任何部分服務。

3. 若因個人因素取消、改變意願或無法完成施作，本店不退還已支付費用；若已施作項目費用超過已付金額，您需補足差額。

4. 一經開始施作，視為您已確認設計與效果，不得主張與施作前溝通不符。

三、施作前告知

預約即表示您確認：

1. 您目前無任何血液傳染疾病（如 HIV 等）。

2. 您無心臟病、高血壓、血液疾病、嚴重過敏、凝血功能不良、傷口癒合不良等會影響施作安全之情況。

3. 若本店評估不適合施作並建議延期，但您堅持施作，需自行承擔所有風險與後果。

四、施作後須知（非常重要）

為確保效果，上色及避免感染，施作後您需遵守：

1. 保持傷口乾燥與清潔。

2. 七日內避免接觸水、蒸氣、流汗、游泳、桑拿等。

3. 十五日內避免日曬、辛辣、海鮮、酒精、色素深食物。

4. 結痂請勿摳抓、勿自行剝落。

5. 配合本店提供之修復方式。

6. 上色不均、微紅、微藍、結痂狀態均屬正常現象，顏色會逐漸回復。
   若未遵守上述事項導致效果不理想，本店無須負責。

五、補色服務條款

1. 補色僅限本店原客戶於指定期限內預約。

2. 逾期則視為放棄免費補色，後續補色需以自費方式另約。

3. 補色當日遲到 30 分鐘以上視為放棄免費資格。

六、影像授權（可選）

預約即表示您同意本店可於 不揭露個資、不影響隱私 的前提下，使用施作前後照片或影片作為行銷、作品展示或教育用途。
如不同意，可於現場主動告知。

七、免責聲明

Dear Dream Beauty 將以專業技術、安全衛生為優先。
若因個人體質、傷口管理不當、或未遵守術後照護導致：

• 色料不著色或上色不均

• 結痂狀況異常

• 過敏、紅腫、感染等反應

• 效果與預期不同

均需自行承擔後果，本店無須負民事、刑事或行政責任。

By booking or receiving any service from Dear Dream Beauty, you acknowledge that you have read, understood, and agreed to all terms below.

1. Personal Information & Health Disclosure

By making an appointment, you confirm that:

1. You have truthfully disclosed your skin condition, previous semi-permanent makeup history, allergies, and any relevant health information.

2. You do not have any conditions such as severe allergies, blood disorders, thyroid issues, diabetes, heart disease, hypertension, healing disorders, or any illness that may affect treatment results or safety.

3. You are not pregnant, breastfeeding, or on your menstrual cycle on the day of service.

4. You understand that withholding or providing false information may result in complications, for which you take full responsibility. Dear Dream Beauty is not liable for any adverse reactions caused by undisclosed personal health conditions.

2. Service & Payment Terms

1. All services provided by Dear Dream Beauty are cosmetic in nature and results vary depending on individual skin type, metabolism, and lifestyle.

2. The service will only begin once full payment has been received. No partial service will be provided before full payment.

3. If you cancel or cannot complete the service due to personal reasons, no refunds will be issued.

4. Once the procedure has started, you may not request design changes or claim dissatisfaction based on pre-service expectations.

3. Pre-Procedure Acknowledgement

By booking, you confirm:

1. You do not have any blood-borne diseases such as HIV or hepatitis.

2. You do not have medical conditions that may affect the procedure, including:
   – heart conditions
   – high/low blood pressure
   – anemia
   – bleeding/clotting disorders
   – severe allergies
   – keloid tendency
   – diabetes
   – immune or skin disorders

3. If the artist advises postponement due to health concerns and you insist on proceeding, you accept full responsibility for any resulting complications.

4. Post-Procedure Care (Very Important)

To ensure proper healing and the best results, you agree to follow all aftercare instructions:

1. Keep the treated area clean and dry.

2. Avoid water, steam, sweating, swimming, or sauna for 7 days.

3. Avoid sun exposure, spicy food, seafood, alcohol, and dark-colored foods for 15 days.

4. Do not pick, peel, or forcefully remove scabs.

5. Follow all aftercare products and instructions provided by the studio.

6. Uneven color, redness, or temporary discoloration during healing is normal.

Failure to follow aftercare may affect the outcome. Dear Dream Beauty is not responsible for poor results caused by improper aftercare.

5. Touch-Up Policy

1. Complimentary touch-up is limited to the designated validity period only.

2. Late arrival over 30 minutes will be considered a forfeiture of the complimentary touch-up.

3. Expired touch-ups require a new paid appointment.

6. Photo & Video Consent (Optional)

By booking, you consent that Dear Dream Beauty may use procedure photos or videos—without revealing personal identity—for portfolio, education, or marketing purposes.
If you do not consent, you may inform us prior to the service.

7. Liability Waiver

Dear Dream Beauty follows strict sanitation and safety standards.
However, you acknowledge that:

• Healing varies by individual

• Color retention is not guaranteed

• Results may differ from initial expectations

• Temporary reactions such as redness, swelling, peeling, or uneven color are normal

You agree that Dear Dream Beauty is not liable for adverse reactions or unsatisfactory results caused by:

• personal skin type or medical conditions

• failure to follow aftercare

• natural fading or uneven pigment retention

• previous unknown or undisclosed cosmetic procedures

You release Dear Dream Beauty from any civil, criminal, or administrative liability arising from the procedure.
